Three Cases of Inferior Vena Cava Obstruction associated with Hepatoma

Abstract

|
Inferior vena cava obstruction is a relatively rare disorder, which is characterized by dilated superficial abdominal veins with blood flow of cefalad direction and edema of lower, extremities.
We report 3 cases of inferior vena cava obstruction associated with hepatoma. All of them showed characteristic clinical manifestations of inferior vena cava obstruction and their sites of obstruction were confirmed by inferior vena cavography. In two cases, the obstruction were just above the point where hepatic vein entered inferior vena cava and in another one, from common iliac vein to right atrium.
All the cases also had characteristic clinical findings compatible with hepatoma; hepatomegaly of hard consistency, bloody ascites, cold area on liver scan and positive aifa fetoprotein.
It is suggested that mechanical and functional obstruction .of inferior vena cava had been developed by underlying liver cirrhosis, which progressed to hepatoma with aggravation of inferior vena cava obstruction.
